Intake forms

Print and complete your intake before the appointment, or come 15 minutes early and Carmen will get you set up with a paper copy.

Personal injury or attorney lien patients: we handle personal injury cases every week and work with most major personal injury attorneys. Use the personal injury intake above, and tell us at scheduling that it is a personal injury case so we document everything correctly from day one. Bring your attorney's contact information and any imaging from the accident.

What to bring

• Photo ID
• Your insurance card (if you have one)
• Any imaging you have had done (MRI or X-ray discs welcome)
• A list of medications
• Comfortable clothing (gowns are available if needed)

Insurance

We accept most major chiropractic insurance. About 60 to 70% of our patients pay cash for simplicity. Spinal decompression is typically cash-pay. We are transparent about what your visit will cost before you arrive.

Hours

• Monday: 7:30 AM to 11:30 AM and 3:00 PM to 6:00 PM
• Tuesday: 8:30 AM to 11:30 AM
• Wednesday: 7:30 AM to 11:30 AM and 3:00 PM to 6:00 PM
• Thursday: 3:00 PM to 6:00 PM
• Friday: 7:30 AM to 11:30 AM
• Saturday and Sunday: Closed

Note: Thursday is afternoons only, 3:00 PM to 6:00 PM. If you need a morning visit, Monday, Wednesday, and Friday start at 7:30 AM.
